POLICIES & PROCEDURES

CLUB HOURS

The club hours will be posted on the window/door at the Elkader Fitness Inc.

 
TYPES OF MEMBERSHIPS

Memberships will be a commitment of one year. The first payment is due upon the signing of the contract.

Semiannual contracts are due six (6) months from the date of this contract.
Quarterly contracts are due three (3) months, six (6) months and nine(9) months from the date of the contract.

 

SPECIAL MEMBERSHIPS

At the discretion of the Board Special Memberships may be offered throughout the year. This includes but is not limited to college student holiday specials.
Trial Membership: A one month trial membership will be offered to any individual who would like to try Elkader Fitness Inc. The cost of the one month membership will be $50.00. They would be required to sign a contract. This would be a one time offer. If they decide to continue the membership, $25.00 will be deducted from their annual membership fee. A new annual contract must be signed and paid for within 15 days of the expiration of the trial membership contract to receive this credit. (4-29-2009)
Non-Resident, College Student and Visitor: Non-residents or visitors to the area, will be offered a contract to use the

RECLASSIFICATION OF MEMBERSHIP

Single members who become a couple may do so by paying the appropriate change fee and presenting a new signed contract to the Fitness center.
In the event of dissolution of a relationship, each person is required to pay the dues of a single member. They will need to present a new signed contract to the Fitness center.

 

TERMINATION OF MEMBERSHIP

Voluntary Resignation: A member who wishes to terminate their membership because of unforeseen circumstances such as severe illness or job loss may make an application to the Board of Directors for a change in their one year contract.
Involuntary Termination: Any membership may be terminated by the Fitness center for non-payment of dues or other indebtedness, for violation of the center rules, or for conduct which the center deems offensive or inappropriate. Notice of termination shall be given to such terminated member in writing. A hearing may be granted at the sole discretion of the Board. No further dues or assessments shall occur after termination by the center, but the individual whose membership is so terminated shall remain liable for all sums due at the time of termination.

 

CORPORATE MEMBERSHIP

Elkader Fitness Inc. will offer discounts to the employees of Businesses and/or Companies who contribute to the Fitness center. The discounts are as follows:
$500—–5%
$1000—-10%
$1500—–15%
$2000—–20%
This discount is on a single membership only.
Proof of employment must be provided.

 

MEMBERS – RESPONSIBILITIES
  1. To protect the equipment and facilities, street shoes must be removed upon entering the facility. Members must wear a separate pair of clean gym shoes.

  2. All members are asked to sign in when they arrive and sign out when they leave.

  3. To protect the equipment and improve sanitation, all participants are asked to wipe off equipment after each use. Spray bottles of sanitizer and towels are available at the center.

  4. Return all equipment including weights to their proper place when finished using them. Unload bars when you finish with them. Do not leave weights on the floor.

  5. Do not drop or throw weights to the floor.Do not sit on machines when you’re between sets; allow others to use them.

  6. Be patient when waiting for equipment, and be efficient when using equipment while others are waiting for you.

  7. Observe the time limits on equipment if others are waiting.

  8. If any piece of equipment is not working properly, please place a note on the bulletin-board and on the equipment.

  9. Members should communicate any concerns or suggestions to a member of the board.

  10. All members are required to read and sign a membership agreement and waiver.

 

GUESTS
  1. A responsible adult (18 or older) must attend children under the age of 16. The responsible adult must be within arms length of their child at all times.

  2. Children who use the facility are required to be covered by a membership.

  3. Limit for the number of guests per member is two. If an exception to the guest list is needed, please contact a Board member at least one day in advance.

  4. Members are responsible for the conduct of guest(s) and any destruction of equipment and/or the center.

  5. Out of Town guest fee: $5.00 per person/per visit.

 

CHECK-IN PROCEDURE

All members will be issued an electronic FOB to enter the center after signing a contract. The cost of a FOB is $10. Your $10 can be refunded if FOB is returned at the termination of your contract.


They will also be asked to sign-in and sign-out at the front door.

 

ATTIRE
  1. All members and guests MUST change from street shoes to closed-toe athletic shoes after entering the building. No bare feet, stocking feet, cleats, sandals, slippers, water shoes or heeled shoes.

  2. Shirts must be worn at all times.

  3. Belts that can damage the equipment are not allowed.

 

CONDUCT POLICY

The following conduct or behavior is not permitted at any time within the facility:

  1. Disorderly behavior of any kind, including excessive noise, running in the building, or otherwise causing a disruption to the peace and order of the fitness center.

  2. Vandalizing or defacing materials or property including equipment, furniture, walls or any other aspect of the fitness center.

  3. Removing materials, equipment or property from the building. No equipment is to leave the fitness center.

  4. Possession of a camera phone in any of the areas.

  5. Using a cellular telephone, audible pager, audio equipment, audio-visual equipment, or computer equipment so that it disturbs others.

  6. Indecent exposure, voyeurism, exhibitionism, or other lewd and lascivious acts.

  7. Bringing firearms or any other deadly weapons into the fitness center.

  8. Harassment of other fitness center guests or staff, either verbally or physically. This may include verbal abuse, use of

  9. Profanity or other abusive language, intimidation, staring at or following another person within the facility, sexual harassment or harassment on the account of race, religion, ethnic background, gender or sexual orientation.

  10. Smoking or use of tobacco products.

  11. Possessing or consuming alcohol or illegal drugs or being under the influence of alcohol or illegal drugs.

  12. Selling products or services; soliciting for personal gain; or approaching guests within the building for the purpose of obtaining signatures for petitions.

  13. Posting or distributing unauthorized material or literature. Approval must be obtained in advance from the Board.

  14. Leaving personal property in the facility unattended. All property must be stored in a locker. The fitness center is not responsible for lost or stolen items.

  15. No spitting on the floor. Discard gum in the wastebasket.

  16. Closed plastic containers of beverages will be allowed in the fitness center. No Glass or Cans will be allowed.

 

DELINQUENT ACCOUNTS

The Treasurer and the Bookkeeper will apprise the Board of any delinquent members and prompt payment will be requested. If payment is not received within 30 days said member will have his/her keypad number disabled and access to the center will be denied to all delinquent members until the balance is paid in full. To reinstate the keypad, there will be a fee of $10.00 assessed.(7/21/2009; (10/6/2009)(3/2/2010)
Upon reapplication of former members who have defaulted on their contract, unless previously approved by the Board, the option of paying quarterly or semiannually will no longer be offered. The new contract must be paid in full before a new keypad number is assigned.(7/21/2009)

 

CLASSES

All requests to hold classes at the Elkader Fitness Inc. will be presented to the Board either in writing or in person. Prices will be set and the instructor will sign an agreement with the Board. Instructor will advise class members of any items that apply to them.

  1. Members of the class will not use any other equipment unless specifically requested by the instructor.

  2. The instructor will maintain a list of all the individuals that are in the class and record attendance on a daily basis.

  3. Those taking the class will pay a fee as established by the agreement.

  4. The instructor will be responsible for collecting all the dues and getting them to the Treasurer or Bookkeeper in a timely manner.

  5. The instructor will be responsible for the Waiver completion by each class member.

  6. When classes are cancelled the instructor and/or co-instructor will notify each individual member of the change as soon as possible.

  7. The instructor of the classes will be responsible for following any and all rules of Elkader Fitness Inc.
